Infusion services

Our Infusion Services team is committed to providing the support necessary to ease a patient's transition from hospital to home. We will work with your Community Care Access Centre case manager to coordinate home infusion therapy through the on-site formulation of medications, as well as the timely delivery of infusion supplies and equipment.

Our pharmacists and clinical experts are valuable clinical resources who work with the patient or caregiver and other health professionals to set treatment goals that are regularly monitored and reviewed. Our Infusion Services team is on call 24 hours a day, seven days a week.
